The $30 Holiday Gift Certificate program is now closed! The City of Crystal Lake received over 6,000 applications for our program and our staff members are working diligently to review every application as quickly as we can. We ask for your patience and to allow us 6-8 weeks to review your applications. We are only able to provide $30 Gift Certificates to the first 3,333 complete applications that are received and processed by the City of Crystal Lake, on a first-come, first-served basis. We will provide you with written notification of the results of your application submittal.

2009-10 Holiday Sales Incentive Program Terms and Conditions
1) Gift certificates are available to the first 3,333 applicants, in the order that complete applications are received and processed by the City of Crystal Lake, on a first-come, first-served basis.
2) Complete applications are those that include copies of sales receipts totaling $300 or more (including sales tax) and a completed application form.
4) Limit one application and gift certificate per person. Receipts are valid for only one gift certificate application.
5) Applicants must provide copies of the entire sales receipts. The City cannot accept partial receipts. Receipts must include: 1) The store name and location, 2) All of the items purchased, and 3) Date of purchase.
6) The City of Crystal Lake is not responsible for incomplete forms or late, lost, illegible, stolen, delayed, un-postmarked, postage due, or misdirected mail.
7) Please allow 6 - 8 weeks for gift certificate processing.
8) The gift certificate program is open to both residents and non-residents.
9) Completed forms must be postmarked by January 15, 2010.
10) Eligible purchases are those items for which sales taxes are collected. Receipts for non-taxable professional services, tips and the purchase of gift cards/certificates are ineligible.
Other non-tax, ineligible purchases include:
1) Pharmaceutical prescriptions or co-pays
2) Training and education fees
3) Health club membership dues
4) Entrance fees
5) Medical office visits, including chiropractors and optometrists
6) Cash donations to non-profits or other organizations
7) Utility bills
8) Government fees, fines, taxes, or assessments
9) Extended warrantees, performance guarantees, or product/service insurance coverages
10) Periodicals, newspapers, magazines, or other subscriptions
11) New or used vehicle purchases or leases
12) Postage or shipping charges
13) Lottery tickets
14) Purchases of gift cards and gift certificates
15) Labor, installation, or professional services
